About This Clinic. (Downloadable version)

This clinic is currently staffed by one physician, Dr. Hadi, a nurse, Wendy, and an administrative assistant, Kim.

Dr. Hadi is a Family Physician with a medical degree from the University of Ottawa, training in family medicine from the University of Toronto, and certified by the College of Family Physicians of Canada. He is registered with the College of Physicians and Surgeons of Ontario. In addition, he holds a B.Sc. in Chemistry and a M.Sc degree in Biochemistry from the University of Manitoba.Dr. Hadi is part of the Lynn River Family Health Organization, which consists of a group of eight physicians in Simcoe, Port Dover and Delhi who work together to provide coverage for each other's patients after regular clinic hours.
Dr.Hadi is the only physician in Port Dover who belongs to this group.

Wendy is the clinic nurse, having worked in this capacity for over 45 years

Kim is the clinic administrative assistant She brings over 25 years of experience in her role managing the administrative side and tries to keep everyone happy.

Appointments are also discussed in this section.
When you arrive, please check-in with our receptionist. You will be see on-time, therefore please arrive a few minutes earlier to go through the check-in procedure, and have your blood-pressure taken if needed. Patients must present their health-cards each and every-time on check-in. This is an OHIP requirement.
Patients who are late may not be seen, as they would make other patients who arrive on time late.


Cleanliness of the Premises
We take great pride in keeping this clinic clean for you. This can only be accomplished by having visitors remove their footwear and wear slippers or foot covers provided at the door. This is especially true in winter. You are asked to kindly comply with this request.
If you have a cough, you will be asked to wear a mask to prevent others from potentially getting sick.
Feel free to use hand-sanitizer which is available for you to use when you enter.


Access for people with Disabilities
The clinic is accessible to patient with disabilities. We have disabled parking-spots in front of the clinic and the main door has an automatic door-opener. Counters, doors, and spaces are wheel-chair accessible. The staff will make room adjustments at the time to accommodate wheelchair users.
We have a fully accessible washroom in the back of the clinic.


Medical Charts
This office uses an Electronic Medical Record. The use of electronic medical records is governed by the same safeguards for paper records. Prescriptions are issued electronically for you to prevent loss.


Home Visits
For patients who are very sick (cancer, palliative, very frail), home visits can be arranged. If you are unable to come to the clinic for these reasons, please advise our office staff.


Health Cards
OHIP Health cards have expiry dates, so make sure yours is not expired. Patients are required to either submit a valid OHIP health card, or pay for the services they receive.


Specialist Appointments
If you are referred to a specialist, the office staff will setup your appointment and let you know where and when to go. Specialists often charge patients who fail to show up. Remember, if you cannot attend your appointment with the specialist, it is your responsibility to call the specialist's office to change or cancel your appointment.


Evidence-based medicine / Complete or Annual Physicals
Medicine, like everything else, changes. Many tests that you may have been asked to do in the past may be now unnecessary or not needed as frequently as before. Over-investigating and over-treatment is harmful - it can make you sick, cause anxiety, and may lead to unpleasant and invasive follow-up tests that do not change your health outcome. In addition, it may be a waste of your time.
An Annual Physical is one of those doctor visits that is now considered unnecessary and potentially harmful. Refer to 
this guideline as an example.
Preventative health measures have replaced Annual Check-Ups. They only need to take place at defined intervals. The measure undertaken in our clinic is listed 
here. This is based on most up to date medical evidence.


Travel Medicine
Vaccines (hepatitis, typhoid, etc), and medication for the purpose of travel (e.g traveller's diarrhea, altitude sickness, and malaria medication) is not covered under OHIP. The discussion of medications and vaccines for the purpose of travel, and issuance of prescriptions of the purpose of travel is a service you need to pay for.
For countries where you require Yellow Fever vaccination, you will have to attend a Travel Clinic (at your expense), as only doctors who have a license to prescribe this vaccine are able to provide it to you.


Transfer of Medical Records
By registering with Dr. Hadi, you are a patient of the Lynn Valley Family Health Organization, and signing an agreement that you abide by the rules stipulated by the Ministry of Health and Long Term Care of Ontario for rostered patients. Should you wish to register with another doctor, you are obligated to advise us. If you or your new doctor would like to have a copy of your medical record ("chart") available at this office, you will need to sign a consent form to have your chart forwarded to your chart to your new doctor, or to pick it up directly yourself.
You are required to give no more than 60 days notice to allow us to prepare and forward your chart.
Our receptionist will call you when we receive signed consent from you to transfer your chart to another doctor. You will be required to pay the administrative cost for preparing a copy of your chart, and be required to sign a document to that effect. if you fail to return our call, or pay the fees, no further reminders, calls or letters will be sent to you.

This will be provided at your request, and the note is billable to you.
