Medication Renewals

To have your medication renewed, you are required to make an appointment. Requests though your pharmacy to renew medications will not be accepted.
If you cannot come for an appointment for whatever reason, your pharmacist can renew most, but not all, of your medications for up to one month.

Labwork / Blood tests

There is no lab in Dr. Hadi's office. Laboratory services are provided by Lifelabs. Patients can visit any Lifelabs in the province provided that they have a doctor's requisition and their OHIP health card.

Not all lab tests are covered. If there is a charge for a lab test, it may not be a necessary test, so speak to your doctor first before proceeding. Remember, OHIP pays for what is necessary. More tests are not better, and often do not change the course of management.

Diagnostic Tests - Imaging

If you need to have an imaging test (other than an x-ray), our receptionist will make the appointment for you and let you know where and when to go. For x-rays, you will not need an appointment but you will still need to take the x-ray requisition that we will give to you.
For other imaging you require an appointment that we will make for you. If you cannot make it to your appointment, you must call the imaging facility to notify them. Please do not miss appointments as it can take weeks or months to schedule.

There is no diagnostic imaging facility in Port Dover.

CAT / CT scan:
Patients are required to go to Norfolk General, Brantford General, or Hamilton General Hospital.

MRI:
Hamilton General Hospital or London Health Sciences Centre.

Mammograms:
Arranged through the
Ontario Breast Screening Program. A doctor referral is not required if you are 50 - 72 years of age. Please make arrangements by following the instructions in this form.
